First Visit

On the day of your first visit with us, plan to arrive 15 minutes early so our staff can go over insurance and other new patient paperwork with you. For information about our office policies regarding late arrivals, missed appointments, or canceling your appointment, please review our Patient Policies.

Please print out our New Patient Forms and complete at home before your first visit. Bring these forms with you on the day of your visit with your insurance card (if applicable) and driver’s license or photo ID. Referral Policy: If your insurance policy requires you to obtain a referral to see one of our doctors, it is your responsibility to do so. Please note that any follow-up visits are considered separate visits and may require additional referrals. If you do not have a valid referral, you must reschedule your appointment. If you choose to be seen, we will not submit your claim to your insurer, and you will be held financially responsible for the full amount.
